Have worked in the food and beverage industry for a minimum of 6 months and an average of 25 hours per week minimum.
Have a need that cannot or will not be covered by insurance (see examples below).
Pay taxes through your employer. If you are paid only in cash, you are not eligible to receive assistance.

Once your application is received, it will be distributed to a selection committee.
If selected, the committee will determine your award amount and contact you with next steps.
