Information you’ll need to provide

  • Basic contact information including your name, email, and phone number.
  • Email associated with your PayPal account.
  • Website links to your Facebook, Twitter or Instagram account.
  • Number of kids you have and any other people you are responsible for.
  • Brief description of your job and explanation if you lost your job due to COVID. 
  • Two sentences explaining why you need the $500.
